Practice Closure Information
To Our Patients:
We appreciate the trust you have shown in allowing Dr. Daniel Weber and Dr. Edward Joy to provide for your medical needs. Sadly, we must notify our patients about the unfortunate passing of Dr. Weber.
Dr. Joy will be attending to Medicare patients at the medical practice until March 11, 2024 and commercial insurance patients until March 21, 2024, at which time, our medical practice, Integrity Orthopedics will close.
We know that our patients will likely have questions about what this means for their medical care. Below, you will find key information about your continued medical care. If you have any questions we have not answered here, please do not hesitate to call our office at (708) 429-3455.
How can Dr. Weber’s patients receive care going forward?
Dr. Weber’s patients are currently being treated by Cindy O’Neill APRN, Dr. Joy and Dr. Angelopoulos. They are able to confidently recommend an Orthopedic Surgeon, specializing in hip and knee surgery, if surgery is needed.
Where are our other providers going?
Dr. Joy has joined Oak Orthopedics, a division of Illinois Bone and Joint Institute. As of April 1, 2024, Dr. Joy will continue to see patients at our current location on Monday mornings and all day Thursdays. He will also see patients all day on Tuesdays in Bourbonnais. Dr. Joy specializes in shoulders and elbows, treats fractures, as well as other general orthopedic issues. Appointments with Dr. Joy can be made by calling 815-928-8050.
Dr. Angelopoulos will continue to practice at our Tinley Park, IL location. Dr. Angelopoulos is a pain management physician who treats patients with knee injections, hip injections, gel injections, and also focuses on the spine. For appointments with Dr. Angelopoulos, please call 708-429-3466.
For our Physical Therapy patients, Dan Lang will continue providing your physical therapy treatments for Medicare patients until March 8, 2024 and commercial insurance patients until March 22, 2024, at which time we will transfer you to another PTSIR location where he will be treating our patients. The location can be coordinated with Dan Lang during the upcoming months.
How can patients get their medical records?
Should you want another physician to have a copy of your medical records, please download and fill out this authorization form, which must be filled out and returned to our office no later than March 20, 2024.
The office will be open until March 29, 2024 until 4:00 p.m. for patients to pick up medical records. You can stop by once contacted, between the hours of Monday- Friday 8:30 a.m.- 4:00 p.m. to receive a copy of your medical records. We are also happy to provide your records via mail or fax to any physician you have authorized.
After closing, your health records from Integrity Orthopedics will be transferred to Morgan Records Management, LLC (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. After the Effective Date you may submit a request to the Records Custodian:

Please Note:
- In accordance with applicable state law, you may be charged for the copying and transmittal of the records.
- There may be a transition period from the time we close to the time Morgan Records Management has access to our patient charts.
